Welcome to your summer program at Columbia University.
Students participating in Summer Program at Columbia University can access Medical Services for urgent medical concerns; we are located on the fourth floor of John Jay Hall.
Columbia University requires that all participating students provide documentation of immunity to measles, mumps, and rubella in accordance with New York state public health law. The process for documenting immunity is described on page two of the Columbia immunization form.

Please be sure to complete all relevant sections of the questionnaire. All completed questionnaires should be sent to the admissions office sponsoring your summer program.
