COVID-19 Vaccine Upload Instructions

Please read the PDF instructions below for step-by-step instructions of how to provide documentation of your COVID-19 vaccine.

    Part One

    Enter the dates of your dose/s.

    Part Two

    Upload a copy of your vaccine record in order to complete the vaccine requirement. Lack of or incomplete documentation will result in processing and compliance delays.

    Be sure your uploaded document contains the following information:

    • Date of Dose One
    • Date of Dose Two (if a 2-dose vaccine)
    • Name of vaccine received for each dose
    • Organization or provider administering each dose

    Students receiving boosters or additional doses only need to upload a document. You DO NOT need to enter dates.

    All documentation must be submitted in English or accompanied by a certified translation (at the employee’s or student’s expense).

    Verification of documentation is at the sole discretion of Columbia University. Additional information or documentation may be requested and if requested must be provided in order to verify a submission.
