What's New For 2011-12
Every year Columbia Health re-evaluates the benefits of the Columbia Student Medical Insurance Plan, making adjustments and negotiating changes as necessary so as to offer the most cost-effective appropriate coverage possible. Below are the benefit modifications for the 2011-12 plan year:
Basic Level
• Increased annual maximum for prescription medications from $1,500 to $2,000
• Increased per incident copay/deductible from $25 to $30
• Eliminated $500 annual maximum for physical therapy and implement an eight (8) visit per condition per policy year maximum
• Established a $6,000 annual maximum for out-of-pocket expenses (excluding copays)
Comprehensive Level
• Increased annual maximum for prescription medications from $7,500 to $10,000
• Increased per incident copay/deductible from $10 to $15
• Established $4,000 annual maximum for out of pocket expenses (excluding copays)
Both Basic & Comprehensive Level Changes
Increased prescription medication copays:
• Tier 1: Generic copay from $7 to $10
• Tier 2: Covered Brand Name from $25 to $30 (Preferred)
• Tier 3: Covered Brand Name from $40 to $45 (Non- Preferred)
